For Referring Providers
Refer a Patient
We accept referrals from physicians, therapists, schools, hospitals, case managers and community organizations across Brevard County and the Space Coast. There is no form to fill in. Call, fax, or send the patient to us directly.
How to Send a Referral
Whichever is easiest. A phone call reaches our front office directly and is the fastest way to ask whether we are the right fit for a particular patient.
Call (321) 779-9838
Best for anything with a question attached: whether a presentation is within our scope, which clinician fits, or how soon we can see someone. Our front office can answer all three.
Fax (321) 779-4502
The right channel for records. Send the referral, relevant history and the patient's insurance details, and include a callback number for the patient and for your office.
Have the patient call us
Many referrals arrive this way and it works. Give the patient our number and tell them who referred them, and we will take it from there. New patients complete an initial visit in the office, so please do not send them expecting a first appointment by video.
What We Treat
Our clinicians see 13 presentations across 8 services. Every item below links to what that care actually involves here.
Services
Who Your Patient Will See
Psychiatry and therapy sit in the same practice, which means a prescriber and a therapist can both be involved without your patient managing the handoff between two organizations.
MD & APRN
Prescribers
Psychiatrists and psychiatric nurse practitioners for evaluation and medication management.
LCSW & LMHC
Therapists
Licensed clinical social workers and mental health counselors for individual, couples, family and group work.
5
Age Bands
Children, preteens, teens, adults and older adults, so a family can stay with one practice.
Availability moves between clinicians, so call the office to confirm who is taking new patients before you tell your patient when they will be seen. See the full roster for each clinician's license, specialties and the ages they work with.
What to Include
The referral itself
Your patient's name, date of birth, and a callback number. Tell us who you are and how to reach your office, so we can close the loop with you rather than through the patient.
What you are asking for
Whether you are looking for a psychiatric evaluation, ongoing medication management, therapy, or a combination. If you are not sure, say so and we will talk it through. A referral that names the question gets to the right clinician faster than one that names a diagnosis alone.
History that changes the plan
Relevant treatment history, current prescribers, any medical conditions that bear on psychiatric care, and the patient's insurance. Send what you have. We would rather start with an incomplete picture than wait on a complete one.
Anything time-sensitive
If the timing matters clinically, call rather than fax and say so. A fax sits in a queue; a phone call reaches a person.
Before You Refer
Three things worth knowing, so a referral does not arrive somewhere it cannot be helped.
We are not a crisis service
The office is not monitored around the clock and this website is not monitored in real time. A patient in immediate danger needs 911, a hospital emergency department, or the 988 Suicide and Crisis Lifeline, not a referral queue.
First visits happen in the office
Telehealth here is for follow-ups. Every new patient completes an initial visit in the office, so a patient who cannot travel to us at least once is not a referral we can start. After that, most of our clinicians offer video follow-ups for therapy and for medication management, and the patient needs to be physically located in Florida for one.
Coverage is worth checking first
We are in network with most major plans, and the accepted list is published rather than described. If your patient's plan is not on it, call before you refer and we will tell you where that leaves them.
Referring a patient
Call the office and we will take it from there
Our front office can tell you who is available, whether a presentation is within our scope, and what your patient should expect. If it is easier, fax the referral and we will follow up.
